INTRODUCTION

Pike High School (PHS) issues each student a technology package: a Chromebook, charging cord, and protective carrying case.  PHS’s Chromebook Self-Insurance Program has been established to provide parents/guardians/students the option to purchase protection covering the repair or replacement costs of the Chromebook. The purpose of the Chromebook Self-Insurance Program is to provide families coverage at a very low price. Families are strongly encouraged but not required to participate in this voluntary protection program.  Although not required, it is financially advantageous for parents/guardians to purchase the protection plan.  At a minimum, just one repair of an unprotected Chromebook exceeds the protection plan cost.

Families who purchase insurance from an outside vendor, or choose not to purchase coverage at all, are responsible for repair or full replacement costs of the PHS technology package.

Care of all instructional materials, including Chromebooks, distributed by PHS is the responsibility of the student to which they are assigned and his/her parent/guardian.

Terms and Conditions

POLICY PERIOD

The Chromebook Self-Insurance Program is effective August 1, 2020 and will remain in effect until August 1, 2021   or the date the student withdraws from PHS, whichever occurs first.  Policies purchased at any time between those dates will terminate on August 1, 2021.

The Chromebook Self-Insurance Program may be purchased anytime during the school year and will cover repairs made from the date of payment forward (i.e., coverage is not retroactive and does not cover repairs made/assessed prior to payment of the Chromebook Self-Insurance Program).

Students who are issued devices at the beginning of the regular school year may purchase insurance until the end of the third week of school based on the board adopted calendar without having the device inspected. After the end of the third week of school the device and charger must be inspected by site library staff before coverage can be purchased. If devices are not issued during the first two weeks of school the grace period may be extended at the discretion of the school administration for up to three weeks after device distribution.

COVERAGE COST & REPAIRS/REPLACEMENT COSTS

The Chromebook Self-Insurance Program may be purchased through PHS for $25.00 (non-refundable) per year by the parent/guardian.  A Chromebook Self-Insurance Program purchased mid-year will not be prorated.  The coverage is due within 10 (ten) school days following the purchase of coverage, or in special cases, alternative payment arrangements may be made, waived, or reduced for certain eligible students.

The Chromebook Self-Insurance Program covers up to four incidents of covered events to the Chromebook per policy period.  Each claim will necessitate a deductible; this is separate from the $25.00 coverage payment. The deductible applies to each loss. Then PHS will pay any amount over this deductible up to the limit of insurance for this coverage.  Deductibles are due within 10 (ten) school days following the notice of claim.

Students, who have one or more insurance claims during a policy period, for their regular, loaner, or replacement Chromebook, will face increased deductibles per incident as follows:

*Product costs are subject to change based on actual cash value at the time of loss or damage but not more than the amount required to repair or replace.

Fees will be assessed at replacement cost to cover damages to peripheral items such as,  power  adapter,  case, and  other  components  or  accessories  used  with  the  Chromebook, at the end of the year if damaged

PHS will pay for the cost of repair of the insured device to include parts and labor. If the device cannot be repaired, an equivalent replacement of PHS’s choosing will be provided. If are replacement device is provided, this coverage will transfer to the replacement device for the duration of the policy. While a student’s device is being repaired the student will be issued a loaner device of PHS’s choosing. The policy will cover the loaner device until the student’s original device is returned or a permanent replacement device is issued. PHS has the final say in determining insurance and repair situations.  Insurance pricing is reevaluated at an annual basis, prices are subject to change.

COVERED EQUIPMENT

The Chromebook Self-Insurance Program applies to any Chromebook issued to your child by PHS, whether it is the originally issued Chromebook, a loaner, or a replacement. Power adapter, case, or other accessories are not included.

REFUNDS

There will be no refunds. If a student is unenrolled from PHS for any reason, the Chromebook Self-Insurance Program null and void on that date. There are no refunds once this Self-Insurance Program becomes active. If a student withdraws from PHS and then re-enrolls later in the same school year, the Chromebook Self-Insurance Program will be reinstated until the expiration date of the original policy period.

CLAIM PROCEDURES IN THE EVENT OF LOSS OR DAMAGE

All Chromebook Self-Insurance Program claims must be reported to the library staff within 24 hours of notice of the occurrence. In the event school is not in session, the student must notify the librarian via email.

In cases of theft, vandalism, or other criminal acts, a police report, or in the case of fire, a fire report must be filed by the student or parent for the protection coverage to take place. A copy of the police/fire report must be provided to the library staff before a Chromebook can be replaced with the Self-Insurance Program plan. In the case of intentional damage, the student/parents are responsible for full payment.

FRAUD, CONCEALMENT, AND MISREPRESENTATION

Coverage may be denied if the student willfully defrauds, conceals, and/or misrepresents any material information about the cause of damage or loss of the device.

DETAIL OF COVERED EVENTS

  • Accidental Damage - Damage from drops, such as cracks, dings, housing damage, or shattered screens. Liquid damage, from spills to full submersion.
  • Mechanical Breakdown - Mechanical and electrical failures. Defective buttons or connectivity ports.
  • Theft, burglary, robbery (requires official police report)
  • Vandalism (requires official police report or school administrator incident report)
  • Fire, flood, natural disaster

Loss, Theft or Damage

By taking possession of technology package (a Chromebook, charging cord, and protective carrying case), the parent/guardian/student agrees to assume full responsibility for the safety, security, and care of PHS’s property.

In the case of loss or theft occurring at school, the student must report the incident to the library staff within one school day of the occurrence. In the case of loss or theft occurring away from school, the student must report the incident to the law enforcement officials of jurisdiction within 24 hours of occurrence and then provide documentation of the law enforcement report to the library staff.

Failing to report theft in the manner described here will result in the missing property being categorized as lost and the student/parent/guardian will assume full responsibility for the loss of the device and the corresponding financial obligation for the replacement costs of the lost property.

EXCLUSIONS OF COVERAGE

PHS will not pay for loss or damage caused by or resulting from the following:

  • Loss or damage caused by or resulting from corrosion, rust or changes in humidity or temperature. Cosmetic damage which means damage or changes to physical appearance of the covered property that does not impede or hinder the normal operational function of the scheduled property such as scratches, abrasions, change in color, texture, or finish. Damage that does not affect the functionality of the Chromebook is not covered.
  • Loss or damage caused by dishonesty or any loss or damage arising from illegal acts whether committed alone or in collusion with others. However, if the loss is caused by an act arising out of a pattern of criminal domestic violence and the perpetrator of the loss is criminally prosecuted for the act causing the loss, this exclusion does not apply to an otherwise covered loss suffered by another insured who did not cooperate in or contribute to the act that caused the loss.
  • Loss or damage to electrical equipment caused by electricity other than lightning.
  • Loss or damage that is caused by or resulting from theft from an unattended vehicle except when it is securely locked, its windows are fully closed, and there is visible evidence that entry into the vehicle was forced.
  • Loss or damage where the only proof of loss is unexplained or is caused by the disappearance of property without the knowledge as to place, time or manner of its loss.
  • Government seizure.
  • Loss or damage caused by or resulting from wear and tear, gradual deterioration, insect or vermin. Wear and Tear means the reduction in value to the Insured Product stemming from routine use and exposure.
  • Loss or damage to the software and data.
  • Tampering with, voiding the manufacturer’s warranty by altering the software, or unauthorized attempts to repair device.
  • Consumables (the USB-C charging cable or the case).
  • Not informing the library staff within 24 hours of damage to the device may void the insurance coverage for the incident. 

INDEMNIFY

The student and parent/guardian release, hold harmless, defend and indemnify PHS from any claims, liability or money damages (including attorney fees) brought by a third person, the student or the parents/guardians against PHS and related to the student’s use of the device or the student’s or parents’/guardians’ breach of these terms.

CANCELLATION

This policy may be cancelled at any time in writing submitted to the library staff.  Written notification must include the date which the coverage will end.  The date cannot occur before the date of written notification.

When cancellation is for nonpayment of premium, PHS will give at least 10 days’ written notice of cancellation accompanied by the reason for cancellation.

STATE LAW

Any part of this policy which conflicts with the laws of your state is automatically changed to conform to Indiana law.
